Well, you might wonder what have actually happened?
Today, 28 Mac 2013,
I had a practical exam for Skill Development: Dispensing module.
This test is to evaluate our skill and knowledge
in prescription screening.
We are supposed to identify the potential drug drug interaction,
address it to the doctor with a valid and evidence-supported explanation.
Then, we will have to suggest alternative drug of choice regarding the interaction we'd found.
Well, I managed to identify the interaction.
Sadly, I can't understand what was the explanation stated in the BNF...
Thus, I gave a totally wrong explanation...
I checked the National Antibiotic Guideline, found the appropriate suggestion.
However, the drug available in the pharmacy trolley doesn't match with the suggestion I found in the guideline.
Should say, I did not put much attention to that, in fact, I do know it was entirely different medication.
Or perhaps, I assume the choice of medication stated in the guideline was the medication I found in the pharmacy trolley.
I should have rechecked and double confirm the medication I found and the medication I suggested.
Should have read more carefully when I was reading the guideline and BNF.
I should have been more calm and careful and pay more attention to all these detail.
It is a matter of life and dead.
Should have not assume anything instead, do more readings.
Guess what? The dosage we suppose to dispense was 250mg twice daily, instead of that, i dispensed 625mg twice daily.
How careless was I!? -_-..
That was a mistake that a pharmacist should not make.
That was a mistake that I personally cant bear with and yet, I did that.
Sigh. I can accept if I fail the test.
As that was totally my mistake, and it was a big mistake.
Thank God for allowing me to see the weakness in me.
I will definitely learn from my mistake, and do better in the future!
I will definitely get myself well prepared with the knowledge and communication skill!
